1st Trip to Italy

We will be traveling to Italy 9/26/2015 through 10/10/2015. We are taking the train from Rome to Venice and spending 2 nights and then taking the train to Florence for 2 nights. after that we plan on spending 3 nights and 4 days in the Tuscany, Umbria and Chianti regions. We have not traveled outside the United States before and hoping to be prepared.

My questions are:
What hotel would you recommend in Venice that would be near St. Marks Square and convenient to other sites.
What hotel would you recommend that is walking distance in Florence to the most popular sites.
What city would you recommend staying in as a base location to visit the Tuscany, Chianti, and Umbria Regions.
